Allow system to automatically send email notifications for upcoming appointments

For upcoming appointments, with certain Activity Types, it would be good to give the user the option to send automated email notifications based on pre-set time increments like 3 days, 1 day, 4 hours and 1 hour. Add the option to the Activity Type Lookup maintenance screen.
